This session will focus on the diverse community within south western Sydney. This session will be chaired by Dr Bernadette Brady and feature presentations focused on prevention and mangement of chronic disease, the impact of peer-mentoring on CALD patients’ engagement, and specific projects in refugee populations from:

This session will focus on our diverse older population within south western Sydney. This session will be chaired by Professor Friedbert Kohler OAM (Clinical Stream Director, Aged Care and Rehabilitation, SWSLHD) and feature presentations focused on the life-course approach to optimal ageing, the burden of frailty and what constitutes ‘brilliant’ aged care. 2:10-4:30pm | Innovating for our Community (Chaired by Mr Matthew Jennings) | |
The call for abstracts is open until 28 July to submit abstracts for rapid-fire presentations that focus on local projects for the south western Sydney community. The day will end with a technology demonstration, leaving attendees feeling invigorated for the opportunities now and into the future. | ||

This session will focus on locally led projects to improve outcomes for our Aboriginal community. The session will start with a performance from Muggera Cultural Enterprise and presentations on projects such as the Fairfield Hospital falls prevention program, Patient Reported Measures in Cancer Services and the Aboriginal GOTIT Program. This session will feature presentations from:
